LONE STAR MAT CLUB

LIABILITY WAIVER AND ASSUMPTION OF RISK AGREEMENT

This document is a legal agreement between you ("Participant" or "you" or "your child" if a minor) and Lone Star Mat Club/Lone Star Mat Club Foundation, Inc. and its affiliates (collectively, the "Club"). Please read carefully before signing.

ACKNOWLEDGMENT OF RISKS

I understand and acknowledge that:

1. Wrestling is a vigorous sport involving severe cardiovascular stress and physical contact.
2. Participation in wrestling and related activities carries inherent risks that cannot be eliminated even with the utmost care, including but not limited to:

○       Serious neck and spinal injuries that may result in paralysis

○       Brain damage

○       Injuries to bones, joints, muscles, and internal organs

○       Broken bones, sprains, bruises, concussions

○       Physical and mental trauma

○       Death

3. Additional risks may include:

○       Travel to and from activities

○       Participation at sites remote from medical assistance

○       Potential reckless conduct of other participants

○       Inadequate protective equipment

ASSUMPTION OF RISK

I KNOWINGLY AND FREELY ASSUME ALL RISKS ASSOCIATED WITH PARTICIPATION, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE of the Club, its instructors, other participants, or anyone else involved in Club activities.

SAFETY AGREEMENT

I acknowledge that I have been informed of and understand all safety rules and procedures. I agree to comply with all safety instructions and to immediately report any unusual hazards to Club staff.

MEDICAL CONSENT

I confirm that I have no physical, mental, or medical conditions that would endanger myself or others. I have consulted with a physician prior to participating in these activities. I consent to receive necessary medical treatment in case of injury and accept full financial responsibility for any medical or evacuation costs.

RELEASE OF LIABILITY

TO THE FULLEST EXTENT PERMITTED BY LAW, I HEREBY:

1. RELEASE, INDEMNIFY, AND HOLD HARMLESS the Club, its officers, employees, contractors, facility owners/operators, and any organization conducting activities (collectively "RELEASEES") from any and all claims, demands, losses, and liabilities arising from injury, disability, death, or property damage WHETHER ARISING FROM NEGLIGENCE OR OTHERWISE.
2. COVENANT NOT TO SUE any of the RELEASEES for any reason related to my participation in Club activities.

This agreement shall be effective for each and every instance in which I participate in Club activities, now or in the future.

GOVERNING LAW

This agreement shall be governed by the laws of the State of Texas. Any disputes shall be resolved in the state and federal courts located in Tarrant County, Texas.

BY SIGNING BELOW, I ACKNOWLEDGE THAT I HAVE READ AND FULLY UNDERSTAND THIS AGREEMENT. I UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING AND DO SO FREELY AND VOLUNTARILY WITHOUT INDUCEMENT.

By signing this form, I hereby grant Lone Star Mat Club permission to:

1. Capture photographs and/or video recordings of me (or my child) during club activities

2. Use these images and recordings across various platforms including the club website, social media, newsletters, and promotional materials

3. Share selected content with media outlets and partners to promote club programs and events

4. Include images in marketing, advertising, and communications to showcase the club's activities

I understand that:

• No compensation will be provided for the use of these photographs or recordings

• These images may appear in electronic or print format

• Lone Star Mat Club will use these materials responsibly and professionally

This consent remains in effect until explicitly revoked in writing to Lone Star Mat Club.
